Medical Marijuana: Am I Eligible?

All 4 of the following criteria are required to register for a medical marijuana in Connecticut certificate through iMed Chiropractic:
- He or she is a Connecticut resident
- At least eighteen (18) years of age or older
- Have, or think you may have, any of the following debilitating conditions:
- Widespread body aches, fatigue, and tenderness (Fibromyalgia)
- Headaches that are difficult to treat, or fail to respond to standard treatments (Intractable Headaches/Migraines/TMJ)
- Facial Nerve Pain
- Degenerative Osteoarthritis and associated spinal disorders that may cause nerve pain (Sciatica)
- Chronic Neuropathic Pain (Nerve Pain)
- Failed Back or Neck Surgery Syndrome with Persisting Pain/Post Laminectomy Syndrome
- Chronic Pain of at least 6 months duration associated with a specified underlying chronic condition
- Ehlers-Danlos Syndrome Associated with Chronic Pain
- All noted above are conditions/symptoms that we would gladly diagnose and treat in-house and do not require a furnished note from an outside physician, unless you are a Medicare recipient.
- *If you are a Medicare recipient you will need to obtain a note from your treating physician stating you have been diagnosed with one of the conditions above.
- Or are seeking a certification certificate that will require a note, with a diagnosis stated on letterhead, from your treating physician for any of the following conditions:
- Muscular Dystrophy
- Multiple Sclerosis
- ALS
- Osteogenesis imperfecta
- Post Herpetic Neuralgia
- Intractable Seizure Disorder
- Irreversible Spinal Cord Injury with Intractable Spasticity
- Cystic Fibrosis
- Cerebral Palsy
- Ulcerative Colitis
- Crohn's Disease
- Epilepsy
- Parkinson's Disease
- Positive Status for Human Immunodeficiency Virus or Acquired Immune Deficiency Syndrome
- Psoriastic Arthritis
- Complex Regional Pain Syndrome
- Rheumatoid Arthritis
- Sickle Cell Disease
- Hydrocephalus with Intractable Headache
- PTSD
- (The above noted are conditions/symptoms that require a note from an outside treating physician)
- Provide Payment for the Certification Evaluation. (Examinations are $160 and annual renewals are $110)
