Medical Marijuana: Prepare for Visit

Before you arrive, we prefer that you complete our registration form iMed Chiropractic offers our patient form(s) online so they can be completed in the convenience of your own home or office.

• BE SURE TO INCLUDE A VALID EMAIL ADDRESS. (This email address is necessary to sign up with the State of Connecticut Department of Consumer Protection).
• Submit any pertinent health records via email: info@imedchiro.com or fax: (203) 366-7588, or bring them during your visit. Examples:
  • Note on letter head from your treating physician stating your diagnosis.
  • Any imaging reports including MRI or X-ray
  • Please include a copy, front and back of your driver's license and health insurance.
